I filled mine out myself and the next letter I received said that I was not under review. They didn't say why. Anyways, that's been my experience. Don't panic! 2.) If you are still suffering from the same issues? Tell them, and add anything that may have gotten worse over time. I started collecting in 2006 after an almost 3 year fight, I have never had a review though. Medical experts submitted testimony that I would only get worse, and I have. Good Luck!

Doesn't it go by my income? And for 1 person wouldn't I be DQ'ed?
