A few years back, I bought a health plan that I later found out was something called a "death spiral." What our beloved insurance capitalists did was sell insurance to a group of people called, "Plan A" and then closed off that plan and began selling to another group of people called, "Plan B" and so forth. Then, as people in Plan A began getting sick and needing their insurance, the insurance company could legally raise the premiums to keep Plan A solvent. Of course, the healthy members in the plan moved on to new insurance rather than paying severely-escalating premiums. That forced the members in the plan who had serious health problems to either pay thousands of dollars per month in premiums, which most simply couldn't do, or drop the plan altogether.
