Though not related to MASH, I've seen several movies where an offical, like SecDef or the President, will say, "Go to DEFCON4." DEFCON4 is business as usual. DEFCON1 is for the Russians are coming over the Pole.
Seen this error 5-6 times, calling a ship to starboard then turning to port.
The destroyer skipper, running down a sub, tells the helmsman, "30 degrees to starboard." Next long shot shows the ship turning to port.
Whaling lookout yells, "Thar she blows, 2 points of the starboard bow." Next picture shows ship turning to left.

One thing I remember is the character named Spearchunker. He was a black guy. But the Army had no black surgeons in Korea.

These aren't mistakes per se, but two things I always thought was off in general especially with the extras. People were too old for their rank, and there were a lot of people who were borderline overweight. The soldiers and young officers should have looked like skinny kids. But then again, everybody on the show was sporting mid 70's hairstyles, so making the show look like it was 1950 obviously wasn't a priority.

True, but it still seems odd by today's standards, how little effort they made to depict other time periods in the shows back then... or other places. All the TV shows at the time looked like 1970's California, regardless of when or where the show was supposed to be taking place. And don't get me wrong, I love the MASH, but when I watch it I can't help but think, "why is everyone so old, fat, and look ready to head out to the disco?"
